First, you need to turn on your phone in boot mode. From power-off, press-hold volume-up, then press-hold power, holding both together for 5 seconds. The Boot Mode selection menu should appear.
Now, you need to put the phone in Recovery Mode. Start by pressing volume down to pick Recovery, then volume-up to select. What you see and press after that depends on the version of Android your phone has.
For Android 5 and up, you should first see the "Motorola powered by Android" white screen, then a black screen, then a dead (or sleeping) Android robot with red exclamation mark. Press-hold power, then press volume-up and release both keys.
Before Android 5, you should wait for an Android logo. Once you see it, press-hold volume-up, then press power and release both keys.
Now the phone should be in Recovery Mode and you should see the menu where you can select "Wipe data/factory reset". The rest is intuitive.
Long press for the on button together with the volume down button and wait until something flash on the screen the select the restore or format option by using the volume up and down key and to select ok by your power key.
