The most common reason for a toilet to continue to run is a problem with either the flapper or the entire flush valve itself. The water is slowly draining so the fill valve keeps running.
The flapper is the least expensive and easiest solution, so start their. It's usually around $5. If that doesn't solve the problem, I would purchase a flush valve kit, around $20, and replace your flush valve.
*Note: If you have to replace the flush valve, you have to take your toilet apart. Start with the flapper.

You should check the 1 1/2" vent pipe if there is one attached to the top of the pump casing. The vent pipe must be vented to the open air, outside of the house so the unit can breath air in and out at all times. A lack of airflow usually creates this type of problems. Leave the vent connection open to the room and flush the toilet, you will see the difference when pumping.

It could be the type of flap you are using. I would suggest you try a different shaped flapper (a cone would work best) and if that doesn't do it, put a new kit into the unit, because it could be that the float system is sticking causing it to continue running.

I have a american standard toilet mode 4112 that keeps running.....any suggestions on how to fix it?

The flapper ball may be contaminated and not sealing or the float level has changed and needs adjustment. You should be able to see if the overflow has water flowing into it continuously.

It sounds like it is running constantly because of the leak. Where is the leak, at the bottom, around the plastic nuts underneath or is there a crack in the tank? Let me know.
Fix the leak and will have fixed the running problem

Either the Fluid-master valve is going bad or your flapper needs to be replaced. Both are an easy fix. To check the flapper run your fingers along the underside of it, if the rubber comes off on your fingers its bad. Otherwise you'll have to try replacing your Fluid-master valve.
