PUK code can only provided from your service provider/carrier. Call them and ask them for your PUK code and enter that into your phone. then it will accept your NEW 4 digit PIN number. Do not enter any more random codes as it will permanently deactivate your SIM card and then you need to buy a new one.
If you have a Samsung SGH - T429 the code to unlock the pin block is **05*29941007* now enter 4 new numbers for ur pin blocked then * the retyped it then press #
