SOURCE: i have a playstation2 console that will not

A few things:
There are numerous problems with the gameshark discs not working with Slim PS2's. If yours is a slim, it's not too surprising.
What is the version of Gameshark you have? Early versions had PS2 identification issues (it often did not recognize it as a valid PS2 disc without many many retries). Gameshark Pro corrected this problem.
Thirdly, you state the disc is 'formatted for ps2'. If it is a copy of a genuine Gameshark, your problems will be amplified.

GOD GARNED N64 GAMESHARK!

The original GameShark for the N64 only works on certain games. During the N64 era, game creators would often include code on the game itself that prevented the use of a GameShark. It is possible that you can find a newer version of the GameShark that would work with this so called "blocked" games, but it might be difficult.

I'm assuming that this is the GameShark Pro for the N64. If so, the button is used to find and create your own cheats, such as infinite life, infinite ammo, etc...

To use this feature, start the game, say a first person shooter, and load into a level and when you are ready to begin playing, press the button. The GameShark screen will show up and you can say "search for code" or "search for changes" something like that and the game shark will run a diagnostic that it shows on the screen. A number will appear noting the number of lines of code executed since the game loaded. When it is finished, press the button again and it will return you to the game exactly as you left it. If you are trying to get infinite ammo, simply shoot once and press the gameshark button again to search for codes again. When it is finished, the number it shows will be much smaller, since much less lines of code have executed since the last run. Press the gameshark button again to return to the game and continue the process until you have only a few, say 5 or less, or even just one code left on the gameshark diagnostic. This code will be the one that dictates the amount of ammo that you have. By altering this code with the gameshark, you can either start with an extremely high amount of ammo, or have infinite ammo entirely. The key to this strategy is to do as little as possible between code searches so that you can quickly narrow down on the code that you trying to alter. Hope this helps.

I turn it on, it shows the gameboy, nintendo sign, and goes blank

Gameshark and other cheat codes have a tendency to mess up or freeze the games. This is usual.

Mission

It could be one of many problems.
1. Are you using a cheat device, Gameshark Codebreaker, Action Replay, ect?
If so, the problem is a faulty cheat code.
Go through the code list and shut off/delete any weapon modifier codes.
That should solve that problem.

2. Is the disk dirty, smudged, fingerprints on it, scratched?
If dirty, smudged,fingerprints.
Rinse the disk in mildly warm water (roughly room temperature) and wipe thoroughly with a non-abrasive cloth (like you'd use to clean eye glasses).
If scratched, three words for you "Dawn Dish Liquid" follow the previous steps but prior to rinsing add about 2 table spoons of Dawn Dish Liquid to the surface of the disk and rub it in with a non-abrasive cloth.
You can use cheaper dish soap but for best results you should use Dawn.
I don't know why it works, I just know it works.

3. Is the laser in your PS2 dirty?
Even if it isn't it is a good idea to run a dvd cleaner through it after about every 10 or so hours of use.
