HI You should be able to call your cell phone carrier, ie AT&T, T-Mobile, etc and have them reset it.  I have had this happen before and they were able to reset it for me.  Call from a different phone so you can fix your phone while on the line.   Also, its a good idea to have your serial number ready for them just in case.  This can usually be found under the battery. PUK stand for Personal Unblocking Key, this is the key that could only generated by your phone service provide. Usually, your 1st PUK come with your phone card, please all the documents you received when subscribe to your provider. If not available, the only think to do is go to your nearest service provider customers service office and they will help to generate a new PUK to unblock your phone.

you have to phone 07973100450 from a land line. it will ask you for your phone number a few times then it will pass you to  a REAL person who will ask you ask you a few security questions like what are the last four numbers of your top-up card and vol lia that is how you get your four digit id code!!!!!!!
