You should try a factory reset, just google the make and model for instructions to do the factory reset, if that doesn't clear the issue you need a new phone.
You could try to reset the iPad. Press the home button and the on / off button together and hold them till the light goes out.about 5 seconds. Then the apple logo comes back and in within one minute the starts screen should reappear.

Just re-install the software again and your are done with this issue. Use the recovery disc that came with the computer or use a new Operating System disc be sure it is Windows that originally came or compatible with your computer.
