This could be a Windows Registry setting that disables you from writing to your flash drive (or formatting it). If you are running Windows after Win XP SP2, you can change that setting.
To disable the write protection:

    1.From the Start menu, click "Run", type "regedit", and press <Enter> or click <OK>.
    2.Navigate to HKEY_LOCAL_MACHINE > SYSTEM > CurrentControlSet > Control > StorageDevicePolicies.
    3.Double click on "WriteProtect" and change the value to 0 (WriteProtect = 0 for disabled; 1 for enabled).
    4.Close the registry editor and restart Windows.
    5.After Windows starts, test writing to your USB drive.
    Important Note:
    Serious problems might occur if you modify the Windows registry incorrectly. **PLEASE** make sure that you follow these steps *exactly* and *carefully*. For added protection, back up the registry before you modify it in case you have trouble so it can be restored.

Dont worry about being prompted, that is an OS option and you may of turned off by accident. When you insert the flash drive into your USB port, does th elight on the drive ligh up? If not your flash drive may of gone bad.

If it does light up, click on "My Computer" and see if it shows up there.

Yes, you will need an adapter unless you have a card reader that accepts that specific type of card.

Accidentally switched my sandisk memory to 'lock'
Locking the card simply protects it from writes. It does not wipe out anything that was on it already.

If you're asking about a Memory Stick, there's a switch on the back. Slide it away from the "LOCK" position. If you're asking about an SD card, there's a switch on the side. Slide it toward the metal contacts.

There is usually a switch on the adapter you are using that is there for write protection. The Micro sd card does not have one. Make sure it is not in the lock "position". See this website for more on memory cards and common problems. http://www.retire-asia.com/microsd.shtml I cut a little piece from the site that might help you
"There is no switch on the Micro SD card itself, but a standard-size-SD adapter (which the MicroSD needs to be plugged into for use with a card reader) and SD cards such as the SanDisk 4/8GB have a sliding write-protect switch similar to a 3.5 inch floppy disk. Data cannot be written if the slider tab is in the 'lock' position. The tab could be damaged or missing altogether. This is probably the first thing to check if you have an SD card write error problem. Using sticky tape (Scotch tape etc) to cover the switch slot might solve this problem, but it may need two or more layers before it works. Some SD adapters need the tab to be placed somewhere between the lock and open positions before data can be written to the card."
