Install fexplorer in phone memory. Then look in c drive of phone, you will find a file called mmcstore. Copy this file to yor computer and open it with notepad. At the end of this file yo will find your memory card password.

You will need to reformat it but not by yourself. You will need to send it to a phone shop to get it reformat. If you do it by yourself, it will be useless.
