I have the same numbers on my blanket, mine has a PAC-215 controller, I found a couple of them on ebay. Don't know it all blankets have the controller so check the numbers on the back of your controller.

BTW have you checked that it's not the blanket? Swap the controls over, if the problem follows the control it's the control. If the problem stays on the same side of the blanket, it's the blanket.

Appears to be that the power is not getting to the temperature controllers. Suspect a problem in the wiring between the wall outlet (if not the wall outlet) to the temperature controllers. Look for broken plug at end of cord or a break in the cord between the plug and the controller. Last, you may have to look at the connections inside the temperature controller the power cord goes to in order to see if there is something loose inside. But do this with the blanket unplugged.
