The latest firmware release for that router is found HERE.
Be very careful to follow the instructions; especially I suggest connecting direct cable to your router to insure there are no interruptions in transmission. Since I don't have a USR I can't tell you where the current firmware number is located. However, the usual places are directly on the router itself, perhaps on the bottom or rear. Also check the manual, although usually manufacturers don't use this because many times they supply manuals for several models and don't want dated material. It wouldn't hurt to send an e-mail to USR support, most of these companies are pretty good about responding. My suggestion is to flash your firmware only if you have a problem and no other choice.
If your flash goes bad or is interrupted your router will be bricked.

After trying for a long time i decided to call customer support, they informed me there is no default user name, the only option you have is to reset the router by pressing the reset button for at least a minute than when you go into the configuration ip address you will be prompted to enter a new user name and password

see configuration tab - You will not be prompted to login the first time you connect to the Web User Interface during the initial setup. During the Setup Wizard procedure, you will be prompted to create a user name and password to log on to the router. If you create a user name and password, you will need to use these whenever you want to log on to the Wireless MAXg Router. The user name and password are case-sensitive.

so if you cannot remember the username and password - you will need to reset to fsactory condition and reset all the settings in the router for it to work with your ISP again
the reset button is on the back - RESET Button: To restore the factory default settings, use a paperclip to press in and hold the RESET button for five to ten seconds. If the RESET button is pressed for less than five seconds, the Wireless MAXg Router will reboot.
