    Is it a 64-bit Vista?
did you restart after the install?
try to uninstall iTunes and then install them again. restart before each step.

I would uninstall Itunes, then download the latest 64-bit version HERE and re-install it. There are two different versions, you probably got the 32-bit version by accident. Good luck!

Itunes 8.1 will not open on 64bit vista

If you manually downloaded iTunes 8.1 then maybe you did not select the 64bit version of iTunes. I know that it warns you before installing.

If you let iTunes download the update by itself then maybe it made the mistake of downloading a 32 bit version. I have Vista x64 and mine updated correctly, so it would be odd if it downloaded the wrong version.

Try reinstalling iTunes with a FRESH downloaded copy off their website of the 64bit version.
