if u have razr v3 and u want to unlocked ur phone juz call ur provider and they will give it to u after u requested it...
i have razr and i juz called my provider to ask 4 my puk and then i ask for the subsidy ..after 24 hours they send it to me....

Try mobile2mobile.net, unlockitnow.com or take it to a mobile service center to have it unlock there for a small fee.

Unfortunately, you can't reset without the subsidy lock code.. sorry.
