In the middle of feed opening there is a sensor and it will be jammed. Try removing stuck paper with tweezers and/or soak it with WD-40. Repeat it a few times. This should clear the blockage.

According to the manual, the Electric Version (there was a manual driven one as well) has no on/off switch, and works from the foot controller or a knee switch.
I have the same stupid problem. I haven't found a way to remove the 411 button but I have discovered that you can at least modify that button to another number and not have a ****-dial to 411 and incurring $2 / call (for me). Just use your stylus and tap and hold on the 411 button until a pop up mention comes up. You can then select the "edit speed dial" item. From here, just change the 411 to another number. not ideal as removing the button all together but at least it'll stop having the errant 411 calls which can rack up a bill.
