Usually, on any roofing nailer, the only nail to just drop out of the nose is the last nail of the coil. The nails are held together with two strips of wire welded to each unit. For a nail to just drop out of the nose, the two strips of wire would have to be broken or not totally welded to the nail. Poorly constructed nails will have bad welding or weak or broken wire within the roll. Carefully unwind a coil of nails and check for brittle wire or loose nails. If coils appear to be well constructed, check the condition of the tip of the driver. The driver should have a squared end to cleanly shear the two strips of wire. If the driver is rounded, the two wires will be pulled till they break off affecting the next two or three nails on the coil. Easy to check. Fire one nail, then open latch and examine the next nail to be fired. The head should be aligned with the other nails. If the head is pulled down out of alignment, the driver is not cleanly cutting the two wires. Good luck

SOURCE: Why does my porter cable nail gun FN250B shoot
check the magazine bolts are tight and that the magazine is snug aginst the nose. also be sure you are using 16 gage brads and not 18 gage
SOURCE: I have a Porter Cable
You have some blown o-rings inside the head that won't keep air where it's needed to return the driver back to the top. Porter Cable used to make a driver maintenance kit (PN60011) but it's no longer available through them. You may be able to find it on a site like eBay. If not you'll have to order each o-ring seperately from a Porter Cable/Dewalt service center.
SOURCE: my makita an451 roof nailer
Nailers that use strips of nails (one nail directly behind the other) will sometimes drive two nails at once and is caused by damage to the driver. The nails are so close together that the driver will hit the first and part of the second nail head. Coil nailers, as your an451, use nails seperated by welded wire making it very difficult the the driver to hit the second nail. Usually when a coil nailer shoots two nails, it is acutally the trigger valve that acutates twice (two very rapid fires). Two very rapid fires is caused by faulty trigger valve. I have seen a roofing coil nailer fire one nail and a second nail drops out of the gun. Caused by weak / malfunction of nail feeder or wire used to weld nails together is too thick.
SOURCE: Hitachi roofing nailer shoots every other nail & doesn't sink the nails it shoots
i think you have two different problems , replace the feeder spring, and replace the oring on the driver piston. also make sure that the coil of nails is perfectly round if they are dropped or squeezed tool will not work right
