Call the customer care from some other phone using the same network, tell them that you need puk, they will ask you for the numbers written on the backside of your sim, and they will get you the PUK.

HI If you are asking for pin contact your service provider , he will give you a default pin . If you try pin more than few predefined no of times your phone will ask PUK code which can also be given only by your service provider which is specific for each and every mobile number . Donot try PUK code many times or else your sim will be blocked and you will need a new sim. Contact your sim customer care department over phone. Its free of cost to get the pin or puk no. Go to the nearest service outlet of your service provider they can also help you. There is no other go than the above procedure only your service provider can give it nobody else in this world nor any software.
Every PUK code is unique so nobody can tell you your PUK number, but i will tell you what you can do!
If you still have your packet that your Sim Card came in, you
should find your PUK number in there, if you don't have the packet,
please read bellow.
You must get a phone that is on the same network as your phone
and call the networks Customer Care and ask them for your PUK number,
they will ask you for your phone number, then they will tell you your
PUK number. Once you have done this, you will be able to reset your Pin
number so you will be able to go into your phone again.

if by coda you mean pin code, then get it wrong 3 times, this then blocks the sim but will be the only way to get into your phone, you then call your network provider and ask for your PUK and then enter this and your phone is unblocked.
and it this an att phone cus if it is u can call att custmer care and they will be able to give u the puk number or u can go online and click on phone/devises at the top of the screen and click for the puk number and it will also give u the puk number there and that will unlock the sim card
when it asked for PUK code if you enter correct the phone will work., if its incorrect you have to call your service provider and ask the PUk code for you phone number, they will help you in this
The PUK code is availlable from your network service provider. This number was on the plastic card that your SIM card was attached to. If you don't have it, only your Network Service Provider can supply it to you.
Keep entering any pin and when the phone asks for the PUK number enter it to reset it. The PUK (Personal Unlocking Key) is on the plastic card where your sim was attached.
1. if you've already registered the sim with the network- go to home page on web login to your account click on the help section click on unblock my sim, enter your mobile number & hey presto PUK code appears
