SOURCE: PUK code for NOKIA 6020
Your nokia 6020 doesn't have problem. It's your sim card. If you still have your simcard packaging, the PUK code is written their inside it's brochure. Or you can also go to your mobile phone network and ask its PUK code. Or simply buy a new simcard with the same network company. Thank you

SOURCE: DEFAULT PUK CODE AND PIN CODE FOR N73 ME
No the Default PUK and PIN codes are not set by you, these codes are Service Provider Dependent (SIM)
All you have to so is to call your Service Providers Customer Support (Like of T-Mobile, AT&T...).
And ask them about the PUK and PIN code, they will first Authenticate few things about the Ownership of the SIM card, and will give you the codes, in return.
I Guess, the picture is clear in front of you, Now.
Regards,
Waqar.

You need to get the puk code from the phone company you are with. Sometimes you can get it by going into their website depending on the Company.

PUK code
and PIN Code:
Please
dont try more than 3 times. If you try more than 3 times the Sim will get
locked. And it will ask for PIN code. Both these PUK and PIN code can be given
only by the network service provider. Please call the network service provider
of your mobile. And ask him to provide the PUK code or PIN code.He will ask you
some basic details like name, date of birth for his verification. And he
will provide you the PUK and PIN
code. This is done for free of cost.
Please do rate the solution if the issue is resolved or post a comment for further assistance.
Thank you for using fixya have a nice day:-)
