By spacing, I assume you mean the margins at the centre.
The default margin settings are not the same for the right and left hand side of the page, so the printing on the reverse side is offset from the printing on the first side.
The right margin is always wider to allow for punch holes in file paper.
It is not the printer settings you need to adjust, it is the page layout of your word processing software.
Adjust the margin settings manually. In MS Office 2007:
Run a half test. Put paper in the manual feed tray. Print something, anything When the paper is 1/2 way into the printer, open the cover. Pull out the toner cartridge. Look down at the paper. Does it have a black line on the right side? If yes, replace the cartridge. If no, replace the fuser.

Using a pair of tweezers, the paper scrap can be easily removed from the exit rollers.
Place the machine gently down onto the surface.
If no paper scraps were found inside the machine, please check the paper in the tray. If the paper is slightly curled, the print head will catch the edge of the paper and tear it. To resolve the issue, the paper in the tray must be replaced and continue to STEP 16.

Your machine has the wrong paper in it or the paper sensor is not working properly. There should be a black mark on the top left hand corner of each page. The correct ekg paper and ekg repair is available from Medical Device Depot.
Replace the toner cartridge; this happens periodically, especially with refurbished cartridges. Parts of the cartridge can go bad even though the cartridge itself still has ink in it.
