I think you mean you need a PAC code. You can only get this from your network provider, phone them and ask. I do not think you should have put your phone number on the internet where the whole world can see it.

Go to DIGI Center then ask for it. They might need the ID of the person whom it was registered to. If it is yours then it will be an easy task getting it.

The PUK or Personal Unblocking Key is a PERSONAL code, meaning no one other than you should know it or have access to it. Contact your service provider and they may be able to help, otherwise your just going to have to find all the documents that came with it to find that PUK.

Please call the Customer Care of your Network Operator (sim card provider), they will tell your the code. There's no other way or no means to get the code except from the Service Operator. They might ask your few questions related to the Sim Card, like your name or your address, tell them promptly and they will give you the puk code for your sim card. It's also important to note down the Sim Card Number (number written on the sim card) if in case they ask you the sim number. You will also be assisted on how to disable the security settings.

Note: if you keep entering random/unknown numbers/codes, the sim card will be locked and will be rejected. It'll not be usable anymore. Good luck.
