The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.

Hi PUK code is called as PERSONNAL UNBLOCKING KEY. Your phone will ask for PUK code when you entered the PIN code wrongly several times depending upon the mobile make. The mobile asks for PUK code so that unauthorized persons do not use your mobile for wrong cause. To get PUK code please contact your service provider through phone or go directly to their office. They will provide you the PUK code for your mobile. The PUK code for mobile phone.so that you can open your phone. When you planning to call your service provider you cannot use your mobile as it is fully locked. Please use some other mobile with same network to call your service provider. While you get the PUK code ,the service provider customer care officer will ask for your personal details to check whether the mobile belongs to you only. If they are satisfied with your answers they will provide with the PUK code DONOT enter the PUK code several times wrongly,It will block your sim card permanently. This PUK code is FREE. First of all, the PIN and PUK codes are not phone codes, they are SIM card codes. The PIN (Personal Identification Number) is a code that restricts access to your phone, through the SIM card. If you insert a SIM card that has no PIN restriction, you will not be asked for the PIN code. When you enter the wrong PIN more than five times, it will get blocked, and ask you for the PUK code.
The PUK (Personal Unblocking Code) is used to unblock a blocked PIN. If your PIN is blocked, use your PUK to unblock it.
Tip:
The PIN and PUK being SIM card codes, no one else can provide them to you, not even your phone manufacturer, except your network service provider. If you dont have your PIN and PUK, call your network service provider customer care and they will provide you with both the PIN and PUK codes, for free.

dont try more than 3 times.If you try more than 3 times the Sim will get
locked.And it will ask for PIN code.Both these PUK and PIN code can be given
only by the network service provider. Please call the network service provider
of your mobile.And ask him to provide the PUK code or PIN code.He will ask you
some basic details like name,date of birth for his verification.And he
willprovide you the PUK and PIN
