First, make sure you recieved the offer of 12 months of free Live subscription. Check your order emails for details. Look for any kind of proof that you did recieve that offer. I personally think you must have mis-read that you are recieving that offer. 12 months of subscription is a lot. I doubt Microsoft will give it for free. But, if you have any proof of it you can claim it. If you used coupon codes keep note them down for the next step.
After you do, contact XBOX 360 customer support immediately. These are the numbers:

Explain them what happened, they will need information like your Gamertag, name, address, telephone number and answer to your secret question. They might also need the details of your payment to make sure you recieved the offer and your
XBOX's serial number. You will need to be in front of your console for troubleshooting. They will help you with it further. Take care.
