I would suggest bringing it to a local service provider (best place would be wherever you bought the phone), T-Mobil, Cingular, At&t, Verizon, or whatever service you use and making sure your service is still good, or if there is something physically wrong with the card or phone.

you need to replace your SIM card now the PUK is a PERSONAL UNLOCK KEY code that changes on a daily basis and if you dont call the provider for this and enter the wrong code too many times it "burns" the sim (security feature)
