MY NOKIA 6233 HAS BEEN BLOCKED, AND IT IS ASKING FOR PUK CODE
Your phone asking you for PUK code means that your LINE, and not your PHONE has been blocked.
The PUK code is a collection of digits assigned to your line, for you to input in the event that your line got blocked (mostly due to entering the wrong pin code 3 times).
If you check your original sim certificate, you would see your puk code on it.
But in the event that you cant find the sim certificate, you would have to use another line, maybe a friend's, of the same network as yours, and call your mobile service operators, tell them your problem, and they would guide you through steps to get your line unblocked.
Alternatively, you could go to any of the nearby offices of your mobile service operators, and tender the issue to them in order to be further helped.
