This is probably a Server Guide issue. You may check with one step lower verion and one step higher version of server guide. It may solve issue.
If that doesn't work, you may have to manually install OS without server guide. You can use a either use a floppy drive or pen drive to load controller driver. Once installed, rest of the drivers can be downloaded from IBM site or install from server guide cd

Your DVD drive's laser is malfunctioning.

You have a few options:

1) Decrease the resistance on the laser to increase power, and it may be able to read the discs again. A guide for that can be found here.

2) If that doesn't work, or you do not wish to do that, you can replace the laser head all together. Replacements can be found on eBay. You can follow this guide to replace the laser head.

3) You can replace the DVD drive completely, but it will require taking the original drive's firmware and putting it on the replacement one. The guide for that can be found here.

Drives don't come with user's guides. The manufacturers leave it to users to have basic technical know how of how to format, install and use a drive. But Windows has some help tools in how to format drives. From anywhere in Windows press F1, then type your querry in the index.

If you know the manufacturer of your drive, you could also check their website for downloadable instructions in how to use the drive.
