You have 2 Options :-

1. Dial *#7370# > Press OK > Enter Default Code 12345 > All Set.

2. This Phone have One more Feature.

Go To Menu > Click on Settings > Select Phone > Click on Restore Settings > Press OK > Enter Default Code 12345 > All Set.

Hi.have you change the Keypad IC (Filter) ? Refer the image which is available in this link and follow 5130 key pad .Good luck

If you enter the PIN code three times incorrectly one message will be shown "ENTER PUK" Then if you again enter the puk 10 times incorrectly"SIM REJECTED"
After these your sim-card will be completely damage.
Therefore, just throw away the old Sim and get new one.
