your puk code is from your sim, not from the phone. You have blocked the pin number. You can find your puk code on the Sim package when you bought it. Or you can call your operator and I think they will tell you your puk code after you will answer at a few questions.
If you insert puk code 10 times wrong, your sim card will be blocked and you can't do anything with it. Just to buy another one and pay for the same number.

You have an software problem, it must be rewritten. The fact is that the free (cracked) SETool doesn't do it, you should buy the box for it, that costs around 250USD. So, better go to a cell phones service center to fix it.
