Disconnect the leads from the 12". Put a voltmeter across the speaker terminals and check for continuity. If it reads open, the speaker is dead.

Why not un-complicate the circuits by eliminating the soundboard first?

Pedals operate at a different level then rack units that go in the effects loop. It needs to go in front of the amp input. Guitar to pedal to amp input.

Normally on older Peavey amps the grill is on a frame attached to the face held on with velcro. I suspect you have something vibrating against the speaker if it's only one frequency. Once the grill is off it may be apparent. Also on old Scorpions and Black Widows Peavy has a piece of foam in the hole in the magnet to keep junk from getting in the speaker. Over time it crumbles and opens that hole, but even worse can get it the gap and freeze the voice coil. I always suggest cleaning it out while the speaker is out of the cab.

I think you may have a bad input jack, or it has come loose from the circuit board.. If you need to take it to a repair shop, ask about their bench charge. It may be cheaper to just replace the amp.

simple !! most peavey mixers have a monitor output !! you can use that to feed new amp , and theres your volume too !!! or if your already using the monitor output , you can buy a split cable (1 in 2 outs) for main send !! this cable would have to be kept short or it will induce HUMMM !! and in real extreme cases if your using a low Z send (cant think as to why ) a device known as a (direct box) normally used by instrument sends can be used in reverse to "tap" into a low Z send and output with a unbalanced send for an amp to your other room , but hey why not just add a speaker with a volume control to it for this smaller room ? and forgo the new amp
