Hi, The problem seems to be a piece of debris , paper stuck in the ADF roller assy. You can check this by opening up the adf cover and inspecting the paper path. I have seen where a very small peice can cause paper path problems to where something is blocking the paper> Good Luck!

When scanning with the ADF all I get is a white
You could be feeding the paper upside down.
Try turning the sheets over and re-scan.
The paper travels into the scanner from the top and then onto the glass. This means the paper should start out with printing face up.

I would do a manual uninstall and make sure to do the device utility at the end and follow the directions step by step from this link.
Redownload the drivers and make sure they are for 10.3.9 and the full feature software to make sure the right drivers are going to be installed.
Also I would unplug the unit while it is on and plug in directly to the wall to make sure not a power issue. The printer does not share power well and pulls it in large amounts - when it can't get it it throws little power fits and sometimes will not turn on at all or it gives error messages.
