The most common issues for duplicate image is either
the drum or fuser. If you are able to measure the distance between the first line and its duplicate, i might be able to narrow it down.

Place a few papers in the tray.
Open up the top, pull out the main (green handle) thingy.
In the very bottom you will see a bar with a single large round rubber wheel in the middle, and white gears on both ends of the bar.
Manually turn the right side gear toward you.
As you do this, you will see the paper lift up and as the rubber wheel turns you will see that it has a notch on the bottom side.
Turn the gear, and watch the rubber wheel until the notch on the rubber wheel is facing up and away from you (and the paper is fully engaged).
Then put the green-handle thingy back in, close the printer, and it should work.
Apparently this resets something.  I have done it a number of times - usually after a paper jam.

This issue is caused by the drum starting to fail. the Drum is what transfers the Toner to the paper and when it fails will cause streaking, lines and fading issues.

right now, i dont know about the cliking noise, there can be many things causing this. but for hevy paper open cover, take out PC drum, under that there is a take-up roller, pull the little knob in right side and then move it to the right side, then you can pull out the take-up roller, then clean the roller and the plate, whats under the roller. installit back. if thats not helping, then you have to take the roller out again, and cut off two plastic sides.

Not an expert on this brand of printers but it looks like dirty feeder rollers to me. Try cleaning them and Print a Blank document and see if the marks show up

Other than the printer just being completely broken (which is VERY likely by the sound of it) the only other thing that could explain this behavior is the computer using the wrong printer driver. Did you yourself install any drivers manually or did you just plug in the printer and Mac OS X installed everything for you?

I just got off the phone with KM tech support for the same problem with my 2430dl.  Take out the drum and remove the toner cartridges, then look inside the printer.  You will see a piece of glass about 6" long.  Next to it, on the right side of the printer, there is a little white plastic enclosure that contains three spring-loaded contacts; this is the sensor that reads the chips on the individual cartridges.  First, try cleaning the three contacts with a swab/alcohol, as well as the chips on each cartridge.  If this doesn't solve the problem, then the problem is likely with the sensor.  The part number should be 4139-6306-02, and it runs about $5. This should solve the problem.

The laser beam is dimmed by leaking powder. Open the top-cover. Remove the drum-cartridge with the green handle. Clean carefully the small glaa-strip deep down inside the printer. Replace the drum-cartidge. Close the cover. Problem solved.
