for PUK code u have to call the service provide of the sim card and they will give u the PUK code or just go to the nearest gallary

You will get the PUK code from your cellular operator itself. So please contact the customer care of the cellular operator and they will provide you PUK code. They will ask for mobile no and the last paid bill or last recharge on your phone.
