If you are referring to an unlock code that relates to the phone itself, like a protection 4 digit PIN to get into the phone, then you will have to reset the phone and probably contact the service provider to get it reset.
If you are referring to the unlock code that unlocks the phone from its service provider then you will have to call T-Mobile at 1-800-937-8997 and provide them with your phone's information and they should be able to provide you with the unlock code.

It sounds like you need to replace your battery. Take your phone in to one of your cellular provider's retail stores and have the battery checked. It's probably defective. Just to be on the safe side, remove the battery from your phone and keep it out until you can have it tested.
