If the bottom thread is loose, it is the top tension that is at fault. Check the tension discs and make sure that dirt etc.. is not stopping them from closing fully to grip the thread. clean them out with a small brush making sure that the foot lift is up, which releases the tension on the unit and allows cleaning.

on most machine the tension for the bobbin thread is on the case either tighten or loosen the screw to ur specfications

Hi, general rule of thumb:: loops on the top, problem on the bottom.. Loops on the bottom, problem is on the top.. When threading your machine, stop just before you thread your needle, put the presser foot down. Try pulling the thread. If it pulls easily, the thread is not thru the tension wheel right. If it pulls hard, you got it right.

With the pattern dial set on the straight stitch, you can align the needle by turning the width dial and setting it to about 2.5 ( so that the needle will stay in the center).

First be sure to follow the steps described in the manual for attaching the buttonholer.

Second, check that the buttonhole lever is pushed because it returns to it's unpushed state after a buttonhole is completed and therefore it will stitch in the same spot.

Also, try adjusting the stitch length. I've found that I can't make buttonholes while on the satin stich setting ( that little rectangle) but if I move it just a bit towards 1 it will manage to make the right stitches.

About the needle problem, Brother's FAQ gives the following response: 

Make sure the needle is correctly inserted with the flat side facing the back and pushed up into the machine as far as possible.

The needle could have been damaged or bent. If so replace with a new one.

Make sure the fabric is not pulled excessively when sewing. Let the feed dogs move the fabric. Do not pull the fabric.

The combination of needle size, thread size and fabric is incorrect. 
Be sure to use the correct needle and thread size for the fabric you are sewing.(example: Denim-size 16 needle and 40-50 heavy duty thread).

To select stitches 11-21 you position the stitch length dial on the "SS" setting , and the pattern selecting dial on the desired stitch number.

You oil this machine anyplace that it moves and rubs. Needle bar, joints that drive the needle bar,end of each shaft, presser foot bar,hook race etc. Do not use 3 in 1 oil. Use a good quality sewing machine oil.

Hi, the only reason I can think of is that it might be the fan belt that moves the arm that is causing the problem. Best to take it in to a professional to look at it for you. Good luck

Some machines do not have a centered needle as a default. That is just how they were made. If she can reposition the needle to the center, then that is a pretty good option. Most older machines with the offset needles default do not have a centering option. (I have an old Nelco that has an offset default--works just fine but I just have to remember that when stitching, the seam line will not be in the center of the presser foot.)
