Have you checked the diagnostics with a simple Scanner yet? This sounds like an Oxygen Sensor going or gone bad. The Scanner will tell you. If the Scanner says 'all normal', and the fuel system indicates 'CLOSED' status, check fuel pressure with a fuel pressure gauge. If the fuel system indicates 'OPEN', check the Fuel Safety Switch wiring, inside and out, the Fuel System Relay, and the Fuel Pump. Sorry, but the ethanol in the gas is creating an entirely new class of problems.

2004 Chevy Silverado 2500 hesitates when driving and then won't start.

I would just about bet a weak fuel pump. With a scan tool connected you can monitor your fuel pressure thru live data. Low fuel pressure will not set a check engine light except if the O2 sensors register a lean condition.

This could also be a faulty fuel pump relay. I had a Honda Prelude that would not restart when hot, but if I let the engine bay cool down for 10 minutes, it would start no problem.

Get it scanned again. If you bought cheaper ones at the parts store instead of oem ones from the dealer you are probably going to be doing them again. The upstream and down stream 02 sensors communicate with each other so it could be the upstream sensors even though it says downstream. I ran into this problem myself.

Van dies after 10 min drive when pulling into

Try removing the idle speed motor on side of throttle body, and spray it clean, as well as the port it came out of, with throttle body spray cleaner. Don't pull and extend shaft of idle motor, just leave it in position it's in, and make sure you don't lose small "O" ring on idle motor. Re-install and try it. If it cures it for a while, then starts to do it again, replace the idle speed motor.
