Sounds like the timing chain / belt has slipped. If it has a timing belt, did you replace it at the recommended interval? If it is a belt, you should get a new belt, install it and reset the timing of the cam and crank.
If it is a chain, then the chain and gear is most likely worn.

If it is a belt and the timing is off, I would not use the same belt. They got out of timing some how. I would bet that when you take the belt off, you will find there are missing teeth on the belt. However, replacing the belt and resetting the timing will put the engine into it's best condition. The real danger is that when the timing is off, the valves tend to be interfering with the pistons. A number of engines are designed to handle this by recessing the valve so that they do not interfere even when the cam is not moving. However, most do not. The most telling sign (without severe damage having occurred) is a tapping sound coming from the head(s).
