At 45 mph my 1989 longbed ford ranger has a rattling sound that comes from the front. At any other speed it is quiet.

Take it to a local automotive shop that you prefer and have them do a wheel alignment that should fix the issue. If that doesn't fix the issue then you may need to look at your struts, wear on the tire, or ball joints.

On acceleration it it misses through 1st 2nd 3rd gear.......runs better when cold... might be egr... disconnected the hose to valve.. still lacks power... put new carb gaskets... thought there might be a...

Sound slike some of the things you mentioned, could likely be causeing the problem. A vaccum leak would steal from the acceleration. also the EGR. You may just need a tune up. New plug, wires, filters, the whole deal.

Ring gear

You already know the axle is 4.10 gears. How did you determine that? Are you just wondering how many teeth the ring gear has? There are 3 ways to figure the ratio, 1st the tag, which is missing, 2nd mark the bottom of the tire, and mark the driveshaft, move the car forward and count the driveshaft rotations in one complete tire rotation, this determines the ratio, which in your case would be 4.10 turns of the driveshaft in one wheel rotation. 3rd would be to count the teeth on the ring and pinion, divide to get 4.10, or whatever it comes out to.

Slight miss in engine, hard shift from 1st to 2nd long 2nd gear

Check both O2 sensor(s) and catalytic coverter.. P0420 means catalyst below threshold. Also check the sensor at the transmission and solenoid.
