1. check fuse for horn.
2. disconnect horn and check with volt meter when horn button is depressed.
if you see voltage then the horn unit cone is defective or not connected - replace or ensure connection and earthing.
if no voltage other problem, suspect horn button is defective.

3. check if security sytem is connected to horn this may be defective and causing horn to no longer operate.

After replacing the radiator, the A/C stopped working and the horn sounds really weak.

I find it hard to relate a radiator replacement to an A/C electrical issue. The horn could be another matter. Many vehicles have dual horns and they are located on the left and right side of the radiator area. It is possible one of the connectors for one of the horns may have been knocked loose, resulting in a quieter and off-tone sound.

theres a fuse that could be blown or a relay thats bad,go to your manual for fuse local, location usually on the inside cover theres a diagram of where that fuse is change,same amps.!!,then try horn if its fixed cool if not then its the relay

Usually there is a relay that responds to opens in the circuit( doors,hood trunk etc) that sends signal to horn and lights to flash.
