Whenever I'm accelerating at about 55-60mph, my engine hesitates & stops a couple of times for a split second then goes. What could be wrong?

I have a similar problem...when at a stop it jerks in hesitation sometime feeling as if it wants to stall. It also does this when accelerating from stop...it has completely stalled out once when accelerating after switching from reverse to drive, but started right away.
