Replace fuel filter if okay sound like low compression.or stopped up fuel injectors.if injectors been cleaned.do a compression test.

If you are doing this with no load on the engine in park or neutral then the engine computers automatic rev limiter is kicking in to prevent free wheel/no load damage to the engine components

My hondacivic 1.6 vtec is misfiring when taking off then it smooths out a bit when sitting still wont rev past 4 rpm

You have a misfire, so you don't need someone to
tell you to diagnose what you already know you have

It won't rev past 4,000 rpm, because it is programmed that way,
just so you don't & ruin the engine
