Does not back up. when going forward makes a high pitch noise, and does not shift to overdrive

Jack the vehicle one wheel at a time...starting with the front and turn the wheel by hand and see if the noise is coming from the wheels...(brakes, wheel bearing, etc)

I really need the year but here is what might be the cause, the engine cooling fan clutch may be defective, if it locks up you will hear a roaring noise that gets louder as the engine speed increases. Open The hood and rev up the engine does the fan make allot of noise, like an airplane propeller?

Possible bearing in one of the drive pulleys. I have changed numerous alternators for this noise. Take the belt off and start the car. If the noise goes away you know it will be pulley related and not transmission related. If noise goes away, spin all pulleys by hand to determine the defective/noisy one

It still could be the throw-out bearing. When going bad, they have a high pitched whine or whistling, and if bad enough, could imitate a vibrating sensation. IDK, I would have a mechanic there listen to it.

HIGH PITCH WHINNING NOISE IS FAULTY TRANSMISSION OIL PUMP.HAVE THE TRANSMISSION FLUID PRESSURE CHECK AT A TRANSMISSION SHOP.IF TRANSMISSION OIL PUMP TOO WEAK OR MALFUNCTION.YOUR TRANSMISSION WONT HAVE ENOUGH OIL PRESSURE FOR FORWARD GEAR , REVERSE GEAR, BAND OR SHIFT VALVES IN VALVE BODY.NOTHING WILL WORK TRANSMISSION.OIL PUMP LIKE HYDRALIC PUMP KEEPS EVERY THING WORKING..

the trubo is a bout to go out

This may sound strange but have someone check out your radio system. Sometimes an antenna wire or wires, when jared loose will make that ear piercing noise. It may not be the problem but it's worth looking into.

Do all the forward gears work or does it rev high in 1st and skip 2nd and shift right to 3rd?

You need fluid!! The high pitched noise yu hear is air in the hydraulic sections of the transmission. Top up the fluid then find out where the leak is

From your discription, sounds to me something vital in your transmission has went out. Pull the dip stick and smell the trans. oil if it smells burnt chanses are you need a rebuild or replacement. Even if the oil dosent smell burnt, with that kind of millage and the shuddering when it shifted leads me to that conclusion.
