It probably comes as a unit. tensioners create tension by way of spring and springs do wear out over time. so it may be beneficial to change the whole tensioner. there is a bolt in the center of the tensioner remove it and on reinstall remeber to line up the dowel pin. good luck

whichever direction the bolt must be turned to relieve pressure on the belt... turn it the other way. it may be reverse threaded if the tensioner holds clockwise tension. if the tensioner pushes counter clockwise, it may just be frozen from heat. try putting steady pressure on it with a breaker bar, and then tapping the bolt with a hammer (GENTLY!!). if that fails, you may need to replace the whole tensioner. these bolts are subject to a LOT of heat, and can get pretty badly seized.
