The next thing is to have the fuel preasure checked, not enough preasure will cause this. also you should have the computer scaned for codes, this is very helpful in finding the problem, may have a misfire or weak coil.start with these things and see what happens,a bad coil pack may be what you will find. hope this helps.

What causes my car ****. could that be my fuel pump

If the jerking happens under load (accelerating on the highway, going up a hill), it could be your fuel pump, but more likely your fuel FILTER.
If the jerking happens at higher RPM, it is more likely a misfire from a bad spark plug wire, fouled spark plug, or bad coil pack,
If you notice a smell of raw fuel when the vehicle is idling, make sure that your air filter is clean.
If the "Check Engine" light is on, have a local auto parts store connect an ODB sensor to determine what code is being generated.

No power on flat roads or hills

Check the EGR Valve, if the problem is only
load related -- meaning mostly under high vacuum
load & up hills

Jerks and looses power on hills and flat land no engine light comes on what can be wrong?

When you said it feels like its not getting enough gas, you may be right.
Since you have replaced the fuel filter, and you are not getting a check engine light, my suggestion would be to check fuel pressure with a gauge. The fuel pump may be weak.

My Van starts to cut out after it gets warmed up or going up a hill, like it is starving for gas

You may try a new air filter and perhaps have your intake cleaned. Now is it "jerking" on you? Perhaps at a stop light when the brake is applied. You may want to address the spark plug wires and plugs.

My 2004 Jeep wrangler has 11000miles it jerks just a little when going up hill, I dont know if its a problem just starting or if its just some maint's it needs. Please help........Scott

May be time to do a good maintenance service on it, Spark plugs, wires, fuel and air filters. More load being placed on the engine on an up hill pull, so this should fix it for you.
