I am assuming you are placing the nozzle all the way into the filler tube, pass the gate?
Using a LED light, and wooden stick (like a chop sick) or non-magnetic screw driver: Depress and open the the Filler Tube Gate. Shine the LED Light down into the Filler Tube to see anything that may be down there. You will be surpised when kids are playing "fill-up" the number of sticks I have found OR the number of rags pushed into the tube by estranged people. If you do see something (only for rags): Try to reach it by using automotive tweezers (available at most auto parts stores) They are elongated, flexible and have pinchers on the end. With sticks however, I do not try to retrieve this way. Too many particles shave off and clog the small opening at the Gas Tank Neck.
Wait until the Gas Gauge is reading below 1/16th of a Tank. Remove the Plastic Splash Guard off the Left Rear Tire. You now should have access to the Filler Tube. Disconnect the Battery. Disconnect the Filler Tube to Body connections bolts. There should be a Support/Guard Plate towards the bottom of the Filler Tube prior to the Rubber Hose, which is connected to the Body. Remove these bolts and the Plate. Again make sure the Gas gauge is reading below 1/16th of a Tank. Jack-up and properly Support the left rear of the Van. You can access the bottom of the Filler Tube under and in front of the Left Rear Tire, where should be a large Rubber Hose. Loosen and move the UPPER metal spring clamps holding the Rubber Hose onto the Gas Tank Neck and the metal Filler tube. The Filler Tube should move relatively freely now, bending at the Rubber TubeS (there is a second vapor tube connected to the tank). Using a non-magnetic screw driver, carefully work the screw driver under the Rubber Hose to loosen the Rubber Hose. Disconnect the Rubber Tube from the Filler Tube. Using a LED light: check up into the bend of the Filler Tube for anything obvious. Use a mirror and LED light check down into the Rubber Hose for anything obvious. You should be able to see down into the Gas Neck opening to the Tank. Check for anything obvious that does not belong there or may be clogging that opening. Dimes (10-cents) are often culprits put into the Filler Tubes.
If need be: Remove the Rubber Hose from the Gas Tank Neck to check this more carefully and/or Remove the items.
