This can be 3 things wrong, either way the inside door trim panel has to be removed to inspect, 1} the glass has broken away from the glue holding it to the frame, 2} the mechanism has lost or has loose bolts holding it to the frame, 3} the mechanism is beyond economical repair and you need to replace the mechanism.
