Follow the upper radiator hose to where it connects to the engine. Disconnect it from there then unbolt the thermostat housing your thermostat should be in the housing. Very unusal you blowing two hoses like that. Make sure your not getting compression blow back from a busted cylinder head that would definitly build up alot of pressure in your radiator.

Temperature goes high without overheating

I am immediately inclined to think that the thermostat you currently have installed is the wrong one for your van. If it isn't overheating, then the thermo is opening at a hotter level than normal. Either that, or the thermo you have is opening late. If you want an easy test to see if this is the issue, remove the hose that goes to the thermostat, remove the thermostat, put the hose back, fill up whatever water came out and drive it around for awhile and see if it made a difference. If it now seems to open cooler, then the thermo needs to be replaced. If that doesn't work and it still seems the same, I would check the water pump or the belt that goes to it to make sure they are working correctly

Could a bad ground be causing my turn signals and blower motor fuses to be burning out?

short in one of the bulb sockets for the backup lights is the most likely cause, water getting into the light is another. the only other thing i can think of is if there is a short in the neutral safety switch.

1999 FORD ESCORT ZX2 radiator hoses swell and burst

you've got a serious overheating issue creating the pressure. Your temp gauge must be reading hot too.
If not, then you have engine compression pressure getting into the cooling system through a blown head gasket.
Replacing the cap will not help.

Yes it sounds like the thermostat is stuck and needs to be replaced. If you have the 4.0 it is a very easy task. Easy to get to also. Good luck.
