In the past we have used starting fluid to find vacuum leaks. Start the engine, let it idle. Spray quick shots of starting fluid at your vacuum lines in isolated areas. When the starting fluid find the vacuum leak the engine will react to it... usually by going up in rpm. Depending on your engine computer, it may stall the engine. Either way you will find your vacuum leak. Get starting fluid or "ether" at Autozone or O'Reilly.

This is a fault for the first O2 sensor, It is important to know the added info scanners provide about the electrical fault detected (can indicate mixtures, heater circuit, slow response or plain electrical fault) Keep in mind depending on the missing info you can have an air leak from the pcv system, evap system or a small hole in the Induction/Intake hoses, What engine do you have?

Sounds like a tell tale sign of a heatercore. Depending on your level of mechanics, lift the hood and check the two coolant hoses going to the firewall for leaks, if no leaks are found, sounds like the heater core is leaking and needs to be replaced.

check the vacuum hose that runs from the brake booster to the intake manifold, if it is cracked or disconnected it can cause this to happen. check all other vacuum hoses and and the intake hose for air leaks. if no problem found replace the fuel filter, the engine may be getting starved if its clogged.
