Honestly a tranny flush, and new filter wouldn't hurt.
I have a 4x4 1999 Silverado K1500 with the 4.8L and had about 180,000 on it when I had the same problem start, after you get into 3rd and OD the problem quits right?
This is an educated guess, but you might want too have your garage checked out the worm gear, especailly if th truck has been used to pull trailers, or been in ana rea where there is slippery roads alot.
It seems Chevrolet had a problem, but no re-call, with the worm gears in that couple of years. The heavy use, and spinning of the tires on wet/snowy/icy roads broke off the spines on the gears.

Bad news.....take your BIG check book to the garage, I paid over $1,000 to get mine repaired (and I know the guys really well).
