I have a 99 ford winstar. When I am driving it jumps alot. It is worse in reverse. But when I stop it is like the transmission is slipping. What could be the issue?

change the fluid and filter to ensure oil flow inside the transmission, if the problem persists, check and adjust the kickdown cable, hope this helps, let me know.
check transmission fluid if needed change fluid and filter when you change filter see if there is alot of metal shavings if so your transmission might be going
Sounds like clutch. The clutch is almost gone and worn unevenly, so if you baby it, it will stay engaged if you put alot of stress on it like up a hill it will slip. This symptom is the warning sign, it will get worse as to where you can't engage any gear. The more it slips the quicker it will fail.
