Could be anyone of the things you have listed , plus , the blower motor it's self could be bad . There are videos on youtube on how to troubleshoot blower motor circuits . First you need to look at a wiring diagram for the blower motor circuit in your vehicle ! You can find free wiring diagrams at http://bbbind.com/free_tsb.html . Enter your vehicle info. year , make , model an year ,plus engine size. Then under system click on HVAC , then under subsystem click on blower motor circuit or something like HVAC controls may pop up on it's own. Click on search , then on the blue link that pop's up. On anything electrical, I ALWAYS check the fuses before anything else. This, of course, requires that you have the manual, as should everyone for all of their vehicles.

My 2005 avalanche heater fan with auto climate control stopped working. The fuses are all good, and the fan blower tests good by hooking it direct to battery.

There is a resistor in the fan circuit to give you different speeds and it burns out. It's down by the fan on most cars.

I have a 1997 blazer... I'm having problems with my heater/fan... today.. it gave out again... about two three months ago (when I was having problems with my wipers) when I opened the hood and was trying...

The fan motor should be on the left side firewall where the heater housing is..it looks like a round cup on a plate that almost is octagonal...usually it has a violet wire going to it..this is the +12 volts lead coming from the resistor module..the resistor module controls the different speeds for the fan assembly..the resistor is a commonly failed component and will caus the fan to stop working...the resistor is on the heater housing near the fan and has a multi-pin connector on it ..it is held in the housing w/2-4 screws....you can get a length of wire and unplug the violet lead from the fan and hook the length of wire to the lug where you unplugged the fan to the positive battery terminal...don't worry about ground as the fan is automatically grounded thru the case of the fan..if the fan comes on the fan is ok and your problem is most likely the resistor assembly...just to be on the safe side...make sure the heater fuse is ok..i would hate to see you go thru all this work just to find out the fuse is bad or the socket the fuse goes in is loose

1997 buick century - blower stays on when car is off

The blower motor relay is stuck, holding the curcit engauged. Look under the hood, find the conveinence center, where you will find fuses and relays (read the cover, see if one says AC/Heater blower relay), if not, look in your car's manual, to see where this relay is located. When you find it, pull it out, see if the moter stops, if ti does , replace it.
