sounds like a bad ecm (computer) may have a solder joint that expands when it gets warm and does not make a good conection.if this is the problem it should run fine when cold (or first starterd)
Determine whether the jerking is engine or transmission related. One way to determine if it is the transmission lock up converter is acting erratically is to down shift out of OD. And see if jerking persist. If it goes away suspect transmission to have problems with torque converter or TCC solenoid . If it doesn't go away determine whether or not the engine sounds to be misfiring. If so vehicle may require a tune up. Many vehicles can be diagnosed for free at your local Auto Zone.
If the check engine light is on, most likely it is take it to Autozone or any auto parts store. They can scan it for free and let you know whats wrong. Or you can get a scanner like this one, its good to have. There are more expensive diagnostic code readers. These will allow you to check all sensors and then you can compare the specs with oem specs. I you plan on repairing the car your self I suggest you get a Haynes Repair Manual.
mcdevito75 here, Sounds like the idle speed motor, Ford is known for these idle speed motors going bad, in the later stages the car races like crazy. have your repair shop check this idle speed motor and change it.
