Vacuum leaks also cause surge and idle issues. After 20 years, vacuum hoses can crack and leak. I'd check and replace any vacuum hoses that look "old".

Both those answers are good. I would check vacuum lines and clean the throttle body and IAC first. Also check the flow from the injector/s. You can look at it and see the flow. Has to be consistent.

Could be the Idle air valve. They get gummed up on ALL vehicles and cause idling and stalling issues. especially when cold.

Idles roughly, then low when stopped or in park, then dies. Decreasing gas mileage. Slow to accelerate

Sometimes that's caused by a vacuum leak near the manifold area.

Sometimes air in the coolant will cause an idle problem. Air will keep the air temp sensor from sending proper info.
