I don't normally see cylinders referred to by letter rather than number. But I assume cylinder D would refer to cylinder #4. The "front" of the engine has the timing belt cover, air conditioner compressor, belts, etc. The engine will be transverse mounted, so the front will be on the passenger side. If you stand on the passenger side of the van looking at the engine, the "front" of the engine is the end closest to you. The #4 cylinder is the second cylinder on your right. For a 6-cylinder engine you count from front to back - left(1), right(2), left(3), right(4), left(5), right(6). In the case of a 4-cylinder engine you simply count front to back - the #4 cylinder will be the farthest back cylinder.

SOURCE: Replacing RH Side view mirror on a 2006 Toyota Sienna

On the inside trim, you can remove screws that are by the door handle, arm rest, under door and side of door and then gently pull back on the panel as there will also be retaining clips attached and you don't want to break them. You'll need to lift the door panel upwards in order to get it off the window ledge. Then you'll need to manipulate the panel and disconnect the door handle linkage from the door handle and disconnect the wiring for your controls. Then you can access the bolts that secure the mirror onto the door
